Interesting office purchase Empty Interesting office purchase

Post by Intercasa Wed Mar 01, 2017 11:33 pm

So over the years I have seen that there isnt necessarily an over abundance of medical life saving equipment lakeside. I am thinking of purchasing a defibrillator to have at my office in case the need arises. I am not sure what is the best brand / model to buy, anybody have a clue? Also would be cool if other businesses bought them too and people knew so there would be more support in the event of a medical emergency and people would not have to go that far when seconds count. I already sent almost all my staff to CPR training when they had it at LCS.

All the AEDs are good, can't think of any that aren't. Anything by Medtronics, Physio Control, Phillips, etc. Good to have, but I think it's unlikely you'll have the need for one, oh, maybe when they get the bill? jajaja Good idea, CR is pretty close and they have them onboard and in the clinic. . .if they're working.

I thought one could not legally give aid to anyone if you're not in the medical profession in some way. Does having the CPR training permit this? I sure would hope so in case of an emergency!

You would have to reach the patient within a couple of minutes from the time of the event. It is more likely to be used where there are large crowds such as airports. You have to ask yourself,"how many persons have had a cardiac event near where I will have the device?" Probably never so the chances of it happening in the future are remote.

A brilliant idea, and one that I have been thinking of for some time, thanks! Not sure if any are available in the area now?

I also bought the finger oxygen sensors, they are cheap and good to see if someone is tired is it because low oxygen levels.

True story. A friend of mine was personal nurse to a high ranking Philippine official. He would travel with him all over the world. They were returning from a trip on a Japanese airline when his boss went into cardiac arrest. They immediately hooked him to an AED. The airline stewards didn't know how to operate the device and the prompts from the device were in Japanese so he couldn't help either. Needless to say his boss never made it alive to Manila.

Interclass, you should have a conversation with a local doctor to see what the average oxygen saturation is in your area. It usually runs over 90% but someone coming in from sea level and not climatized may have lower sats. Those finger monitors will also give you heart rate and that can be a more useful indicator of a person's status

The best thing you can do for someone who collapses in your office, or your home, is check for a pulses first. IF there is none, call 911 and start CPR. THEN consider defib. People who have seizures can sometimes look pretty dead, but will have a pulse, and you do not want to do CPR on someone who has a pulse.

Additionally, if you get an 02 Sat of 87 of someone, what are you going to do? You going to have oxygen too? 87 might be normal for them and if you put them on oxygen and they have COPD you could make them stop breathing all together.

Here is the Chain of Survival from the AHA:

The term Chain of Survival provides a useful metaphor for the elements of the ECC systems concept.

The 5 links in the adult Chain of Survival are

• Immediate recognition of cardiac arrest and activation of the emergency response system

• Early cardiopulmonary resuscitation (CPR) with an emphasis on chest compressions

• Rapid defibrillation

• Effective advanced life support

• Integrated post-cardiac arrest care

The O2 device checks pulse and oxygen, it is for emergencies I want to be prepared as we have a minimum of 15 people coming to office daily, majority over 60 so 75 a week, 300 a month, 3600 per year, it is a high risk group. I was a Boy Scout and learned to be prepared.

Let's not forget, older people dehydrate easily, they become hypotensive, dizzy. Sometimes the best thing to do is to lie them down and maybe raise their legs. You might not even get a good finger sensor reading. Try to feel the brachial artery or the groin artery

But what will you do if their pulse or 02 SAT is low? .

They will come but in an emergency if the person is conscious all minutes count and calling and having them come could waste 10 to 20 even in Chapala versus sending them off in a car and calling ahead.

I really think the benefits of AEM are overblown. It will no help you in situations where you have respiratory distress, congestive heart failure, hypotension, bradycardia, etc. About the only thing it is good for is v-tach and maybe v-fib. And when you have those, cardioversion is only the beginning. You then have give the patient IV fluids, IV medications and have trained personnel available.
Even under the best of circumstances CPR has a poor record of success.
For myself my choice is DNR. Cardiac arrest is not a bad way to go.

Have you ever carried a 200lb unconscious person down a flight of stairs and put them in a car?

Heh, yeah.
Call the professionals, Spencer, they will bring all the necessary equipment and extrication devices to your patient and do everything rapidly, appropriately, and safely. It's what's best for the patient.

I commend you for your efforts and desire to help.
