When you go to renew you'll be given another form to fill out [Formato Basico] and return which has a bunch of questions on it. I'll list them below and then put the English translation.

The form has a place to attach photos (front and right side) and asks for photos 2.5 x 3cm, white background, uncovered forehead and ears without earrings or glasses.

Datos Personales (Personal information)

Primer apellido: Last Name
Nombres: Names
Sexo: (Sex) Masculino (Man) Feminino (Woman)
Fecha de nacimiento: Birth Date Lugar de nacimiento: Place of birth Nacionalidad actual: Citizenship (country)

Pasaporte o documento con el que se identifica el extranjero (Passport or document that identified the foreigner)
Documento de identification: (Type of document, i.e. passport) Numero de documento: Document [passport] number

Datos complementarios (Other information )

Raza: Race
Nivel maximo de estudios: highest education level (college: universitaria, high school: secundaria)
Idioma materno: mother tongue [language] English: Ingles
Area de conocimiento: Area of knowledge [not sure what they want here]
Habla espanol: Do you Speak Spanish [Yes: si No: No]
Numero de hijos: how many kids do you have
Religion: Religion

Media Filiacion: Physical features

Estatura: Height
Complexion Fisica: Build (Choose from delgada [ slender] robusta [ chunky] or mediana [ normal]
Perso: Weight
Senas particulares: Distinguishing features, i.e. tattos or birthmarks, scars

Lugar de residencia: Place of residence

Pais de residencia antes de su internacion a Mexico (country of residence before Mexico)
Tipo de poblacion (Type of city) [aldea: village, ciudad: city]
Nombre de poblacion: Name of city Estado o provincia: Name of city or province
Municipio o sector: Municipality or area

Datos laborales en su pais de residencia (work information in your country of residence)
Actividad principal: Primary work
Situacion en el trabajo: work situation
Ocupacion en el trabajo: Occupation
Sector: Work industry
Ingreso promedio neto mensual en dolares: Average monthly net wages in dollars

Informacion laboral en Mexico: Work information in Mexico
Anos de experiencia laboral en Mexico: Years of work experience in Mexico
Periodo de contratacion (en meses): Time of hiring (in months)

Declaracion del extranjero (declaration of foreigner)

Declarao bajo protesta decir la verdad que toda la informacion que aqui he proporcionado es correcta y completa consciente de las consequencias legales de declarar con falsedad ante la autoridad.

I declare that the foregoing is true and complete and i am aware of the legal consqeuences for making a false statement before a [government] authority.

Hi Spencer,

Perhaps you might want to correct this line:
"Fecha: Date Dia: Day Mes: Month Ano: Year" since ano is not the word for year Very Happy Very Happy

"Raza: Race" -- I assume the choices are White, Black, Asian?

"Area de conocimiento: Area of knowledge [not sure what they want here]" -- I think they're referring to your area of study, i.e., major.

"Estado o provincia: Name of city or province" -- Name of state or province

spencer, today at INM they also wanted to know what county in the US the town i last lived in was located. i'm guessing that went in the "municipio o sector" line.

Yes, estado o provincia is state of province, had a brain fart there, we shall see what else they want, maybe county too!

There is a helpful translation of Formato Basico in English at the Mariners Insurance Mexico site, available from the "Forms" page: Mexico-Formato Basico-English. It is for translation help only, the form is not to be submitted in English. But once you know what information it's asking for, it is easier to complete the official Spanish version in Spanish! (The official Spanish language form is also on that page). There's also an embedded "note" or "help tip" which shows the link to a Mexican government explanation of what each of the fields asks for.

The INM website was updated and the link in the "help tip" mentioned above changed. The new link points to this INM document (links to: http://www.inm.gob.mx/static/marco_juridico/pdf/circulares/04_CIRCULAR_DOF_03-MAR-2010.pdf#page=11). That's the INM circular with Spanish-language instructions for completing FB, for those seeking authoritative reference. The Mexico Formato Basico in English (links to: http://www.marinersmexico.com/documents/forms.php where the updated pdf is listed "Mexico INM Formato Basico-English") on the Mariners Insurance Mexico site has been updated with this new link. Those who have re-posted the previous copy may choose either to update the link in the "help tip" or download the newer version of the document. Thank you.

Rolly, I edited the previous post so the links are also naked and therefore more visible. When they are hyper linked with anchor text it's hard to tell there is a link unless you happen to mouse over the text and it turns red.
